Sussex Primary Care Provider Collaborative

Sussex Primary Care Provider Collaborative

The Primary Care Provider Collaborative (PCPC) brings together providers of primary care services to work collaboratively at a system level. This collaborative approach aims to enhance the delivery of primary care services, ensuring that primary care has a strong and effective voice within the healthcare system. By working together, we can address current issues, anticipate future challenges, and co-create practical solutions that benefit all stakeholders.

Vision

Our vision is to create a sustainable, thriving primary care system in Sussex that aligns with the “Improving Lives Together” mission.

Guided by this, PCPC’s efforts will be to:

  • Gain deep insights into current issues and future challenges in primary care
  • Co-create and deliver practical solutions to these challenges
  • Engage with primary care providers and other key stakeholders
  • Uphold the principles of subsidiarity
